The floating device was put on him and there did not appear to be any rips or tears in the strap when it was secured on him. When he jumped into 4 ft deep water the strap on floating device tore and ripped completely in three places. This cause the device to come up in front of his face causing him to take in water due to allowing his head/mouth to come down lower into the water.
