I buy 5 extra nasal pillows when I purchase a complete cpap DreamWear mask, then I am stocked for a year. **Q: how often does the pillow need to be replaced? Thank You...** A: I have 4 pillows that I rotate monthly.....so each pillow gives me at least 3 months of usage if not more......If the pillow leaks too much, it's time to replace. **Q: do i need the frame also to were this one ?** A: Yes you do. The air travels through the frame to the nose cushion. There is a hose connector on the top of the frame (keeps the hose away from your face) and an elastic belt to hold the frame on the head. This is an extremely comfortable setup and I highly recommend it to anyone using a CPAP. It is made of very "soft", thin silicone type plastic. Very easy to sleep on your side or back. Had a great night sleep on the first try. Does not irritate the nose/face like the typical CPAP masks. One note, there are specific measurements for the frame. Do some research online to get the correct size and complete package. **Q: does this replacement cushion have the nasal pillows that fit into the nose or just fit over the nose?** A: It fits under the nose. I tried the nasal pillow cushions that fit into the nostrils and simply couldn’t use them. I’ve bought several of these. ### ⭐⭐⭐⭐⭐ NO MORE PAINFUL NASAL POLLOWS CINCHED TIGHT AGAINST YOUR IRRITATED NOSTRILS!! YAY *by N***N on December 14, 2016* I HATED NASALPILLOWS THAT HAD TO CINCH UP TIGHT AGAINST YOUR NOSTRILS TO SEAL. THIS JUST GENTLY RESTS AGAINST THE BOTTOM OF YOUR NOSE. AMAZING. I LOVE IT. GREAT SEAL AND GREAT PRODUCT. So, I got the small size set up. I will never go back to any other type of mask. I was new to CPAP and I tried three types of nasal pillow masks. I couldn't tolerate any of them. I even had one where the hose clipped to the top of your head to keep it out of the way. However, the hose protruding off your nose area would pull the nasal pillows out of my nose. I'd have to cinch these tight to keep a seal but through out the night the heavy hose would always pull it loose. My nostrils would scream in pain from those little pads rubbing against them. This mask is a dream. No little pillows pressed up tight. No big hose hanging off my face. This mask leaves me so free. There's just a little slit in the pillow the air flows through so the seal exherts no pressure on your nostrils. Additionally, the hose connects on top of your head so it easily lands in back of my bed out of the way. The comfort level is supreme and it rarely ever becomes dislodged or unsealed. The air flows from the top down the hollow sides of the mask into the opening. You can flip all over the bed without losing the seal and the mask is light as a feather!! I really love it. Love the comfort. It never slips off in the night!! Perfect.
