My colleague has the 27 inch Cardiology III Stethoscope and I feel that the sound quality from mine is a tiny bit more clear and crisp. The 22 inch is short though, and can't be hooked around your neck (like the 27 inch can). It is a heavier stethoscope than average and stays in place well, despite being short. The 27" is mainly for people who want to maintain distance from the patient during the exam or for securing the head of the scope in a coat pocket while wearing the earpieces around the neck, but acoustics will suffer. I like this scope more than my Littman Master Cardiology scope. The diaphragms are versatile and responsive for low and high pitched sounds and the option to convert the bell shaped side to a conventional bell is fantastic for those that prefer it. There are no rubbing tubes to interfere with auscultation and the soft ear tips seal out extraneous noise. In short, the Littman Cardiology IV 22" stethoscope is the most versatile and the most acoustically pure scope yet available from Littman.
