Uses a cable crank mechanism, which is better than a pump (pumps lose traction, the cable crank does not). NOTE: Use two hands on the way down to SLOWLY release, and you will not have a problem. Very easy to use. Has a great carrying case for travel. I was able to avoid subdural shots as this was the mainstay of my recovery program.

A: HI it is hydraulic and it's a great product. I'm a physical therapist and used to use a similar product in the clinic.  I bought this one for home use and it's fantastic.
