It will require you to sit up straighter due to the stiffness around the waist, but I didn’t feel like it was pulling my shoulders back enough for best posture (I have severe scoliosis and my upper back is the problem). It was confusing to put on until I read the directions - do read the directions! I didn’t have any underarm discomfort at all.
