I also trimmed a little of the plastic around the bottom plate where it was rubbing on my gums. I can see now the value in getting a custom one made by a dentist, but I was quoted $1500 for that, so I will stick with my DIY modifications. I saw a lot of reviews complaining about it shifting tooth alignment. This is actually by design and will happen with any Mandibular Advancement Device. It is designed to put inward pressure on your upper teeth and leverage that pressure to push your lower teeth (and jaw) outwards. That is how it's supposed to work. It makes my bite feel a little weird when I take the guard out in the morning, but it fixes itself in a few minutes. If this is concerning to you, then this type of device is not for you. Effectiveness: Does it work? Bottom line, yes, it worked great for me. I have not snored since I started using it. My husband is thrilled. However, three days after using it, I began to experience severe jaw pain. I stopped using it for a few days and then started again; after a few days, the same results occurred. It worked so well with my snoring that I was determined to make it work, so I began using it one day and resting the next. That didn't work, and I started having migraines on top of the jaw pain.
