You need a longer pillow not a taller one so your back has support base to head. There are 3 or 4 brands that do this. A 32 inch length would be great- but 28 inches might be very good. Measure the top of your head to your natural waist. More tan 24 inches? Yup. If you are over 5'10" its almost inevitable. Find the wedge closest to your head waist measurement. Retired massage therapist and pillow selector for a chiropractor. **Q: Do we know if this is 20 inches wide or 24...as we have answers for both? Need wide wedge for knee brace and other leg to fit sufficiently!** A: Mine is 20 1/4 in. wide. It comes packaged in a box all compact and when you open it up the wedge "comes to life" so to speak and takes on it's size and shape. First time for me that I've ordered a product that did that. It's worked out just fine. No loss of shape over the months I've owned and used it. I don't know the science of how that works. **Q: Can one purchase replacement covers?** A: The cover is removable, but I am not sure how to order a new one. We have washed ours. **Q: could this go under yhe mattress?** A: I guess it technically could, but then half the mattress would be lopsided. I think it works best on top as directed.
