Can this filter out worms without hurting them?

Absolutely not. I had to pick them out by breaking up clumps with my hands and removing the worms before feeding soil through.

Will this work for separating decorative stones/gravel?

If the decorative stones you want to seperate are less than 5mm so can drop through the mesh, leaving the gravel in the sieve, or vis-versa, then yes. Its all metal construction so should last well. I've had a problem with wet soil cloging the mesh, so working with stones should be fine.

How well does this deal with clay damp soils etc.

hello it works better on dry soil. But it is a really useful item, i use it every time i weed the garden as we tend to have stoney soil so it helps get rid of these. But damp clay soil tends to clump
