looking for a medicine ball that lays flat on floor for balance testing

This type of ball would not be safe for balancing. You need a balance board or a bosu balance trainer.

What is the diameter of the 4 lb ball. I am looking for 8.5" or so for volleyball setter.

The 4 lb is 6.5" and the 6 lbs is 7.5".

Is this med ball very bouncy?

It does bounce, was looking for something that did not.

Is this ball quality enough to throw against concrete wall? Looking to get for my teenager to gain strength for soccer throw-Ins.

The ball would work for that, but most medicine balls that we have ever seen or sold do not have a cover that will hold up hitting a concrete wall a number of times. Eventually the cover will deteriorate as these balls are not made for that type of use.
