some have holes and some do not... is it random?

Some do havw holes and others don't, it will all depend on which one you select from the options they have.

I wear a size 10 but have a narrow foot. Should I stick with 10 or 9.5?

They fit loose I would stick with the 10

Can these be slipped on?

I returned them. They looked and felt cheap. No cushion at all.
