Can it be worn directly on skin, or should you use with clothing?

I have worn it directly on my skin...no problem...just adjust the temp and timing for comfort

for those who have had this for a while, are you still happy with it? and would I be able to use it on my neck? thanks.

It's excellent for the lower back. Highly recommend. But not suitable for the neck. You could put it round your shoulders, but then you wouldn't be able to move your arms. Try a hot water bottle for the neck.

Can this be charged and used when disconnected from mains supply for freedom of movement

No. It only works when connected to a mains supply.

Using gel microwave pads, but can only use outside dressing gown due to mobility issues, will this be hot enough through a dressing gown ? Thanks

I normally use it over a shirt and it has worked fine. I would imagine it depends on how thick the material is in the dressing gown but I would imagine it would be ok.
