Is the screw plastic or metal?

Sorry, not sure which screw you're referring to. The body components are all metal. Only the parts that come into contact with the guitar are silicone. ( I use mine on a tenor ukulele). Decent quality.

How long has it lasted people?

I have been using daily for the last 4 months with no sign of wear or tear or weakness
