The bolt inside broke.
We used this swivel on a disc swing (that holds 650 lbs) on our church playground. After 6 months of normal use (never exceeding the 800 lb limit) the bolt inside it broke. Luckily the child wasnโt hurt! We are replacing it with a 1500 lb capacity one. Hopefully it lasts longer!
