How does the sizing work? The last time I got a unisex shirt the small fit me perfectly.

I belive it is Unisex :) it doesnt specifically fit me like a men's or women's shirt. But its true to size regardless.

Will a 3xl fit 6’6” 200lb teenager?

It should. Bought for my nephew who is 6'2 and around 250 lbs. It has a nice length good for the tall guys.
