Yes, it's very helpful. I've had issues with my ears for years. (allergies, colds & when I fly) This was worth the price

Not to my knowledge, it just relieves pain by using warm water

I believe it was not intended to be warmed or used that way. However, you can heat the water in the microwave that you must fill it with.

How long do you hold this over your ear?

They can be used as long as they stay warm. Many times we rewarm them over and over because it is soothing to the ear with ear aches.
