Is there a size chart? I’m a 5’11 345 lb woman. What size would I need in this belt?

It tells you the length of the band when you go to the description. Unless something has changed but that's how I determined what size I needed. You need to know your waist measurement.

Will it break down quick? And rip, or tear, with daily use? I have a golds gym one, and i can use it about a month and a half 2 months and it rips.
