Does this bike convert to a two wheeler with pedals, or only a 2 wheel balance bike??

Both are okay. It has two modes. By removing the pedals, it can be converted into a two-wheeled balance bike. Putting the pedals on, it can be converted into a two-wheeled bike with pedals.

Would this be suitable for my 4.5 yr old granddaughter, or should I buy something larger?

No. Don't buy. Buy regular bike instead. He cal also learn how to rdie a refular bike with training side wheels, from walmart maybe. I bought mine. His legs could never reach the paddles although my son was taller for his age. My son has never used this. Its so hars to even paddle outside, almost impossible, mine is sitting up there and waste of money. The front wheel only turns about 30 degrees
