Can you fill it from the top?

No, only the bottom opens. You have to take it off of the kennel to refill it

How does this attach to the dog crate? There should be instructions included.

I hooked it to my kennel door wide.. my dog doesn't make mess with water spilled in kennel anymore.

I have a German shepherd , is this good for large dogs?

It is my belief, this bottle would be too small for a German Shepherd. Although it is perfect for my Shih Tsu, the bottle has to be refilled may times during the day. Hope this answer is helpful .
