Whats the difference between the one size and the large, i.e. at what age would a size large be appropriate for baby?

Frankly, the LONGER the baby is expected to be/is, the larger the size swadle. My granddaughter has a mom who is 6' and a dad who is 6'3", so we knew Loretta would be a 'tall' baby. At 2, she looks like your average 5 year old and wears size 4 girls clothing. My daughter and SIL swaddled her for quite a few months, so you might want to start out with the regular size and then see if the baby is going to continue to be swaddled. These I purchased for my as yet unborn grandson!

How can you change their diaper without undoing the entire thing?

I haven't found a way. There's velcro by diaper area and velcro by chest.

What is the appropriate weight range for this swaddle?

I bought them for my newborn grandsons they we five pounds at birth. They seemed to work fine
