I ordered but under included parts does not show valve assembly?

Everything is included for installation. The valve is the horizontal piece at the bottom of the vertical bar.

Any way to turn both shower heads on at the same time, or only one or the other?

This system will only allow one or the other on at a time.

Does this shower balance the pressure thus avoiding temperature change when the toilet is flushed?

This system does have an anti scald guard to prevent temperature fluctuation when a toilet is flushed.

Does the upper-limit for temperature allow the water to get hot enough?

The valve can be calibrated for the hot water temperatures you would prefer.
