Are these the flat dual forms?

While they are not flat they are much less curved than most other forms. Which is why I ordered them. Hope this helps!

Is this polygel soak off or file off?

It's more similar to acrylic than gel polish. It can be soaked off, but it's easier/faster to file (especially with an electric file).

Does this kit come with the nails forms, metal tool , and cleanser?

Yes my came with the gel the nail forms metal tool with a brush on the other end. Base and top coat and slip solution
