What material is the item made out of?

it is the same material as a plastic tarpaulin. basically it is a tarp with straps sewn in, the straps being the weight bearing component. there are a hundred varieties of these on the market. I have used these in training (though not this brand) and they work very well for emergency evac, but I would get something more robust for every day use of patient transfer.

Would this material slide on the floor or rug? I'm looking for something to place under a mattress that's on a rug so that I can slide it.

It's heavy durable plastic, it will slide on a bed sheet or floor or rug.

Can I use poles to make it more like a regular litter for going down a narrow path?

You could try, I can't see why that would not work. You'd want to roll the patient on first so that the material is flat when you thread the poles through so it doesn't bunch up and you're not rolling them over one of the poles. You'd need sturdy poles.
