Has anyone tried mixing hot and cold foods? Like scrambled eggs at the bottom and some fruit at the top? Do they both stay at the right temperature?

The lid between the hot and cold is well insulated. The top two containers you can put cold stuff or room temp stuff. The bottom two hit stuff. And the bottom two foods are still stwaming hot at lunch time. Just put hot water in the box itself for about 5 minutes.

What is the diameter inside each bowl? Need to know what size bread would fit in. I eat gluten free bread which is smaller than regular white

There is enough room for the jar and the bread. Hope this helps! My husband doesn't like soggy bread, so I pack the filling in the jars and he makes his sandwich during lunch.

What is the country of origin?

The main plant is situated in Japan but the product itself, as I checked the bottom of Mr. Bento, indicates that it is Made in Thailand.

Can the lid be used as a bowl (perhaps a master bowl to put ingredients from the interior bowls into to eat)? How about the main thermos container?

Yes the "Lid" is much bigger than any individual bowl inside. It works great to combine ingredients. You could use the main thermos contaniner but it would be really deep and hard to get food out of and clean afterward. I would definitely use the lid. By the way I love this product for my lunches.
