Q: Can you keep the grains in the basket all the time or do you remove them occasionally?

I have frozen the grains in the basket but the grains grow, so you actually have to remove some from the basket at times, I use that opportunity to wash the basket thoroughly with hot water. If you use soap or put it in the dishwasher, just make sure any soap is rinsed off well.

Q: Does this include the lid?

The container itself has a lid with holes so that kefir can eat but not escape
