will this fit a 4-1/2 angle grinder

It has a 5/8" hole so as long as your grinder has a 5/8" pilot (almost all of them do), it'll fit fine. I've put it in my Milwaukee 4.5" angle grinder with no issues.

Is the diamond media embedded or is it stuck on with glue &/or on a secondary a sheet?

Center hole is very loose for an angle grinder.

The center hole is not the standard angle grinder hole, it's bigger and does not mount tightly. The material looks decent but I have not used much to know if it will last.
