Q: How many trulicity pens will it hold?

I traveled with 6 pens, multiple needle caps, extra lancet needles, and extra alcohol pads with room left over.

Q: how many hours will medicine stay cold?

2 to 3 hours. This is a total waste of money. You're better off with a lunchbox with blue ice . . . but that takes a lot more space!

Q: is this cooler TSA approved for air travel ?

My husband uses it for air travel and no problems.

Q: Can it be used to travel internationally with eye drops that must be refrigerated? Do the ice packs freeze hard and for how long?

I have found it helpful to put the case in the refrigerator before traveling so it is cool when the ice packs are inserted. The longest I have traveled with the cooler is 8 hours and the ice packs were still hard, although I also keep them in a small cooler in the car when traveling. I have not traveled with it internationally. I, too, use the cooler for eye drops that must be kept cold and it has been very useful to have this cooler. You might ask airlines if you can put the cooler in the airline refrigerator/freezer the flight attendants use. Another thought ... can you freeze the eye drops before traveling to lengthen the time it stays cold? I can freeze my eye drops but not sure if yours can be frozen. Check with your dr or the manufacturer.
