Is an 18-24 month olds wrist too tinyy for this?

I would say yes, they likely be too big. They were too big for my 80 pound 11.5 year old. I had to return them.

How small do they go? I have a 7 year old son who wants it.

They are actually fairly big-I got them for 5/6 year olds and they were loose. Many of them wore them as anklets instead!
