Will this work for shoulder pain too?

I don't think so. It is just for neck and nerve pain in the neck and lower scalp area.

Which direction do you align with neck? Do you align the curved side?

The curved side goes along the base of your skull.

Is the effect of this any different from a still point inducer or one of those over-door, hanging-bag traction devices?

I don't know what a sill point inducer is, but it is not like traction devices. It is specially designed for Occipital nerve issues. I didnt find it to relieve anything else.

Will it work in bed or in a chair?

I use it in my bed every night and it works very well. My chair... not so much, but I do have an adjustable bed so I put it in a recline position and it works exactly like I am on the floor or other flat surface.
