Professor Hopkins as I knew is not a technical person, but on this book he successfully explains in clear way how the cheese barrier fall down and how the dependency of each barriers can bring the event into disaster.Very helpful book and I suggest for all field workers and engineers to read this book as a monumental lesson learn.

Excellent book, would highly recommend. Though technical, easy to read and understand. We have purchased these books for our team to read and have found them quite valuable and effective in understanding Human Factors.

As a part of the offshore drilling family, and because I knew one of the 11 personally, I have a strong need to find out as much as I can about this tragedy.This it's, in my opinion, an unbiased account of this disaster and I appreciate the author's monumental effort.

An Excellent book everyone interested in process safety and organisational issues behind major accidents should read

It gives clear organizational context to the Deepwater Horizon event. I great read and less than 200 pages. The author offers various comparisons to other safety incidents.

I found this to be a balanced and insightful book that gave me a good understanding of the numerous factors that came into play.

Worthwhile read

Great book about the attitude of oil company employees and contractors towards process safety and where that can result in.

This book is well reasearched and well written. It tackles the critical topic of process safety extremely well and should be required reading for all in the energy industry.
