This looks exactly like the westinghouse 4500 made in china. is this different and is it made in the us?

Same Generator, just repackaged under the Cummins name. Save the money and get the Westinghouse. I purchased the blue one recently and was impressed for the money. I also have the Honda 3000. This is much lighter and provides more power. The build quality is not as nice as the Honda and probably won’t last as long under commercial use, but for use here and there it is hard to justify spending 3 times as much for less power and only slightly louder. The same week mine came in I purchased one for my father.

So is the 3-year warranty automatic and then you add another 3-year warranty for 120 bucks

The manufacturer provides a 3 year warranty. Any other warranty purchase is from a third party source, such as Amazon.

Product specs say 50 Hz -- that's going to be a problem for many devices in North America, right?

It isn’t a common American cycle, things may run on 220-240volt but not on 110v. Unless specified by the manufacturer to operate on both 50Hz and 60Hz, appliances may not operate efficiently or even safely if used on anything other than the intended frequency.

how quiet is it while running?

just as good as my honda 3000iu gen. My second one of theses with 250 hrs on it without a problem, Good product for the money.
