Does it have the cut out for the logo?

Yes theres a spot for tge logo decal thing

Is this just the cover for the key, or does it include a motherboard?

The one I bought for 34.95 is the entire key, just needs to be programmed and cut. I have not had any problems with mine, it works fine. It's the same as the one from Ford it just doesn't have the emblem on it.
