how much does this sheet weigh?

The weight of the 240x240x1mm carbon fiber sheet is about 109g/3.84oz, 3mm is about 350g/12.38oz; the weight of 300x300x1mm is about 155g/5.46oz, 3mm is about 412g/14.5oz; 400x400x1mm is about 250 grams / 8.82 ounces, 3mm is about 800 grams /28.2 ounces.

How flexible is the 1mm fiber? I need to wrap it around a 5/8" diameter tube.

1mm carbon fiber board is made of strong materials, which is not flexible and can not be wrapped on your 5/8" diameter tube.

Can the 3mm thick material be heat formed to produce a heat shield with 30 to 90 degree bends?

No. Epoxy resins are used to make this CF sheets, and heating the CF sheet will ruin the material. I you want to make custom parts there is woven CF cloth to be formed and apply epoxy mix, for that you will nee to use a mold to give the desire form of what you want or CF cloth that already has resin that can be formed and then heated to harden/catalyze the resin, for the last one you will need more specialized equipment. I recommend find some videos on YouTube to understand and better choose what you want and need.
