does it go flat against the wall and come out when needed, or do you have to pull the table out (a small amount wont be an issue though)?

It doesn’t go fully flat but you could push it back mine sticks out about 10-15 cm from the wall

It's possible to use it upside down? I have a shelf over my desk and I want to hang it on the shelf going down to the table. Is it possible?

Can't see why not. As long as shelf can take the weight of the monitor

whats diameter of the grommet hole need to be?

42mm sounds about right. I expected this to need ~10mm hole to put a bolt through. However, the clamp detaches and you have to feed part of it through your desk and then reattach it. I made a rectangular hole roughly 40mmX8mm. I wish I'd known this before purchasing but it worked fine in the end. It's a different design than most of the others I'd looked at on Amazon
