These forms are good. Very clear and cover everything. Easy to fill out. They will not work if your state requires DOB and AGE.

Missing pages from the consent form only got 39 in total
Shipping was fast! BUT, Just got back from out vacay, opened up all my packages and this certain one is supposed to be a total of 100 pages .. 25 each .. and only got 38 pages total ! Missing part of the consent form, medical form, the 3rd page etc. will change my review if i get what i am missing. Thank you.
