do you have to push the button every time you insert paper?

As a safety measure, you insert the paper, properly align it, then press the button and the holes are punched.

What is the maximum distance between each hole?

I am sorry I do not know the answer as I bought it for a friend for her office. She does the punch at the top of a manila folder, that's all I know.

Does this work on 8.5x11 paper and does it have an adjustable guide?

Yes it works on 8.5x11 paper and yes it has an adjustable guide.
