Miracle piece of plastic
Ive been on CPAP therapy for 10 years. This long term use has caused the development of central apnea, disordered breathing, and etc. I read a research article about this device and decided to give it a try. Best 30 bucks ive ever spent, my breathing was more stable, it was quiet, and after 8 hours on the first night, my machine detected 0 obstructive events and 0 central events. Thats certainly never happened before. I used on my resmed 11 autoset, with nasal cushion headgear. I am a lower pressure 4-7cm with no EPR, and standard response. If you require a higher pressure it may make a little noise? Barely detectable for me. Breathing was comfortable, exhale felt normal. My o2 monitor detected no drops in o2. Incredible results for a piece of plastic, thank you to whomever developed this.

HELP! I'M SUFFOCATING!
Tried the $30 nonreturnable V-Com last night for the first time. Felt like I was suffocating! I couldn't get enough air! Increased pressure up to 16 and still not enough. Exhaling was no issue. After 40 minutes I took it off and felt so much better. I've been using either APAP or CPAP for 9 years and this was the worst experience yet. Only reason I tried it was because of positive feedback all over the web and I noticed my events have been creeping up last few months. I read a review somewhere that said it felt like they were breathing through a milkshake. I agree. Haven't felt breathing that hard at rest since my first round of COVID. I may try again but am very reluctant.

Works well, with some strange but understandable caveats
I saw Dr. Noah's talk on this device at the 2023 Philadelphia Sleep Conference. He said to put it on as soon as you're about to fall asleep. When you're first getting tested for OSA, your fear is having to sleep with CPAP. Once you become acclimated to CPAP, your fear is sleeping without CPAP. Initially trying to use it felt worse than when I first started on CPAP. Like many people, I initiate therapy before I'm ready to fall asleep because I'm terrified to wake up at 3 a.m. not wearing it and not having worn it. In the talk, Dr. Noah warns against this, stating that you should put on your mask just as you're drifting off. That said, I woke up feeling like I could stomp Godzilla. No sinus headache, energy for days, clear-headedness, no feeling of "The best part of waking up--is going back to bed" sung to the tune of that old Foldgers commercial. This is only the first use, so if I have other problems, I'll update the review. As of now, it's looking like a winner. One thing. As another reviewer said, the instructions are very bad. They're meant for physicians, sleep techs, and DME specialists. That said, I have the ResMed AirFit P10 and the AirSense 11 machine, and the V-Com only goes one way between the mask and hose. A nice picture would be nice though, since if you aren't familiar with the terminology, it's a little confusing.

May or may not be effective depending on...
This item may or may not be effective depending on many factors. It reduces the "pulse" pressure primarily, the pulse that causes leaks. To get the same therapeutic value your maximum pressure probably needs to be raised. It is promoted as an aid for new CPAP users to help them acclimate. That may be true, but I am an experienced user. There are no instructions with the unit, other than how to attach it. For the novice, that is probably all that is required, but if you use it, you she observe how it affects your therapy. There is a company in Australia that is currently running a study with volunteers from its member group. This group focuses on quality of SLEEP for people using CPAP. It also has very active user discussions that can be quite helpful. The company was co-founded by a man with a degree in biomedical science.

If you want a less noisy cpap get it
Not sure what the science behind this is but I cannot sleep without it. Makes me cpap less noisy, and also makes exhaling much easier. If you use a mask that connects in the top of your head and you use a climate line hose, get a hose extension and put the v-com between the hose extension and the climate line tube (this is valid for resmed cpaps)

Overpriced, Bad Design, Unuseable
This device is just a simple, extremely overpriced, air flow restrictor, it is not adjustable. In use the device significantly reduces airflow to the mask, far to much, i felt I was being suffocated. The positive reports on YouTube and and elsewhere are from those that have never attempted to use this product and are parroting the manufactures ad copy. Until an adjustable airflow device is created I strongly suggest DO NOT PURCHASE this overpriced, non-returnable, poorly designed device.

Did not work for me. Made breathing in more difficult.
Unfortunately, it didn't work for me. I have a Lowenstein Prisma 20A with a Resmed AirFit P30i mask, and I put the device between the CPAP machine and the hose for the mask. I didnโt notice any difference in noise levels, but I found breathing in harder than before. I tried it over a few nights and also tried increasing the pressure, but that didn't help. In the end, I returned it.

Has made a noticeable improvement to my therapy and comfort. Definitely noisy when fitted top of a P30 mask. So would be best if fitted to the machine end. I use a heated tube so can't because of connections. So now seeking a short extension hose for mask end. Will definitely keep using it.
