How far apart are the holes?

I measure 1/16" between holes or about 3 centimeters. Not sure why it's called 4mm unless it has something to do with the depth of the prongs.

The handle is wood. The wheel is a series of razor-sharp metal prongs. This pattern wheel really will cut through leather and will shred paper patterns if not careful. Appears to be an excellent product.

will this work to perforate cardboard? to make and resize boxes?

Yes, but it's only a pin hole. It won't work as a folding line and is used for marking a line.
