My 115 volt (u.s) monoprice 5watt amp says to use a t500 ma l. but these are rated for 250 volts. does it matter?

It doesn’t matter. It’s the amperage rating that is most important to be exact for fuses. As long as the voltage rating is not below your actual voltage, and the amperage rating is exact, the fuse is safe to use.
