What is the difference between the smooth and flame (apart from the shape)?

Smooth refers to the finish of the handle (Some nail clippers also come with chequered and grooved handles). Whereas, Flame refers to the jaw/blade shape. Flamed Pointed Nippers are designed to cut hard to reach areas around the side of nails.

How can I sterelize them for Personal use please?

It is recommended to always use a sterilizer/autoclave to disinfect your instruments. For personal use at home, you may wipe blades with alcohol after use.

How do these work step by step? I have never used one before.

You carefully position your nail between the cutters (the two sharp bits) and then squeeze the handles (the two long curved bits) between the ball of your thumb and your fingers. The blades should snip through your nail. They work a bit like scissors except that the blades don't cross over each other; they just meet along the cutting line, snipping off a bit of your nail.
