When you check car a/c for leaks, do you have car running and a/c on, or do you check with engine off and a/c off

I have only used this device to check mini splits or associated linesets and have done so when running so the units are pressurized with refrigerant. I suggest you reach out to the manufacturer or a mechanic about how to best use it on a car. WARNING - I would be extremely cautious if using on a running vehicle due to fans and belts moving which can cause serious injury if you are not careful while using this device. There are many businesses that specialize in testing and recharging your car's A/C for about $100 and it typically lasts 2-3 years if you have lost A/C efficiency slowly over a couple of years - very typical in an older car where constant movement allows gaskets to flex and leak slowly (my personal observation and experience). When and why would the replaceable tip need replacing?

Crap can screw up the sensor. Alcohol is likely a good cleaner and I may need a different tip too. I bought another sensor l like for about 6x more. I don’t know how to get another tip.

Do you actually find the led indicator really useful? have you actually seen a case where the leak level is between red and green?

Yes, only on high sensitivity. On low or medium sensitivity where the level should be I have no problem with the led indicator.

Is it sensitive to drops?

Any electronic device is, depending on the height of drop and surface it hits, i.e concrete versus grass.
