is it true to size or run small? I am a 12.

It's true to size based on the sizing chart. Please use that as a reference.

What is a pull on closure and is it easy to take down when going to the bathroom?

This simply means that there is no closure such as a zipper or button. You will just pull it on.

Is the fabric stretchy?

It is a little stretchy, not like gym pants, but there is some give to it.

Can you return this item if it doesn’t fit? At no cost?

You should be able to return to amazon within the allotted timeframe. After a certain amount of time Amazon will not allow returns.
