I can't speak on leather shoe sizes... But I ordered my usual shoe size and they were a tad snug, I suggest ordering a half size larger. Unless you like your shoes snug.

Are the backs of these shoes hard or soft/flexible?

The heel back is structured. But it's more like a hard flexible in it will flatten down if stepped on but it will return to it's shape when pressure is released.
