A customer says "very fragile" are these hollow styrofoam???

There is a hole in the bottom but I wouldn't call it hollow or fragile. It is foam so don't hit it with anything but it can be manhandled with out damage.

Would these break or fall apart if I needed to poke holes through the skull to hang them?

Depends on how big a hole you're looking to put in it. These are pretty sturdy foam, but too big a hole at once and it could force it to break apart. Easiest way to make a hole would to heat up a wire and melt a hole through it.
