Does the high end go in the front of my chair with the u-shaped groove in the back?

Yes, the U shaped area goes to the back. I found if I place the front closest to the front of your chair I get more relief. I really like this product.

Does not sound like this will work for a Toyota Corolla driver's seat? The seat is only 12" wide before the sides angle up.

Can I use it on a leather recliner comfortably?

I use it in my (not leather) recliner and it’s good for me. I wish it was longer in the distance from depth part. I use it every where. But for the recliner I kind of would like it to be a few inches longer just for the thigh/behind the knee area support. It still does the job in my recliner.
