I need one but in a large size, do you have it?

It's not obvious, but at the time I'm writing there are multiple sizes available. You have to click through six identical images (smaller images, to the right of the main, large image) to find the sizes available. Clicking each image changes the size in the description.

how do i determine the proper size as shown in the chart for short, long etc. does this collar fit better than the one size aspen vista ?

Actually, I had difficulty with this as well. My wife took some measurements and came up with it. I was in no position to figure it out as my nect was frozen in place. I recall some instructions regarding the sizing. You might buy one and return it if it is off size or you might just get one at a surgical supply store where you will pay more but be fitted. My wife determined I needed a short. This despite the fact that I am a 6' 300# elephant. There are a few adjustments. I wore this much of the day and to bed at night. Not a big deal. I am out of it now. You should order a spare set of pads so you can swap and clean them. I think the pads are the same for all sizes.

I am confused on if I should order short or regular, neck is approximately 2and 1/4 inches.

If your neck is THAT short, you won’t be comfortable in a regular. It’s meant for a tall, slender neck. I have the regular, but I have a long, thin neck and I have my brace adjustments set as high as they’ll go. Even on the lower size, it’s still quite tall. You can adjust the small to make it taller if need be, but I doubt you’d need to with a neck that short.
