Q: My boyfriend is 6’2 is this gonna be a good size so he can still use it comfortably due to his height?

I'm 6'1" and use it comfortably. Just if your BF is over 350 lbs, be warned the casters can be a bit weak if abused.

Q: All the ball bearings fell out of one of the wheels will you send me a new caster? Ive had it for less than 2 months and only used a couple of times.

I would ask Pro-Lift for a new wheel.

When you checkout, it will state if the item is shipped in manufacturers packaging. You have the option on “most” items to select Amazon packaging besides. Some oversized items may not give this option I have found. This may be one of them.

No, not with the backrest. The pictures shown on Amazon are what you get.
