Yes when I opened amazon box there was the K&N box with filter inside as well as stickers.

Does this specific air filter fit a 2011 gxsr 750? (k&n site states a different model #)

Yes it does I have a 2012 and this is what I bought and installed. The bikes are the same for 2011 to 2014. Not sure if they changed for 2015

Would i need a power commander for this product?

No need. I used it with m4 pipe. Stock ecu. No power commander. Great improvement.
