what is the largest diameter bottle that it will hold?

The description says, "Designed for most cans, cups, travel tumblers & bottles from 12oz to 20oz and most bottles from 1/2 to 1 liter". However, i found the opening for beverage receptacle limiting. The plastic is very rigid and difficult to get coffee cups, or certain small thermos type containers to fit. There is also a rectangular piece that is at the back of opening that gets in the way of an easy fit. Diameter from east to west is 3-1/4"

Will a bottle or cup bounce out if holder is attached to a bike that will be used on bumpy dirt trails?

As long as the bottle or cup fits perfectly in the holder, it will hold. I used this on a long motorcycle road trip recently and it held my water bottle securely. We did go through some bumpy trail, but not for a long time. Still had no issue with the holder. Some coffee cups from Dunkin Donuts or Starbucks may not fit well with this holder. I don't know if those will hold.

Does it come with the antiskid pad or is it sold separately?

It does come with the antiskid pad (as of 11/4/2019). If you didn't receive yours, I would reach out to the company.
