how does it fold up flat? do you need to take it apart to do that?

It doesn't simply fold up flat like other tents. It has poles that connect together so in order to fold it up at all, it needs at least partially disassembled.

Do you have replacement parts for lost pieces

Good luck with that. I didn't even get the proper pieces to make it in the first place. It was a complete waste of money. I had to order another one from a different company.
