My waist is 23". It looks like there's no option if your waist is less than 27"; is that correct?

Assuming that your HIPS are more than 27", you could wear this belt lower or on a slight diagonal, like gun belts were worn in old Westerns.

My dog chewed through the pouch. Is it possible to buy a replacement pouch?

I'm not sure, but you can find belt pouches pretty much anywhere.

Can the waist band tighten to accommodate a petite frame? My waist 24” inch and I’m afraid it’d going to be too big.. I want to use this to run

Yes it goes to any size. This is the best leash. I am able to walk both of my dogs without any problem because if this. Go for it…u won’t be disappointed!

When fully stretched, how far are the handles from your body?

The top handle is still very close to your hand/body. No reaching required!
